Schedule your appointment online through the link on this website above. Pre-payment is required. Insurance is neither required nor accepted. Providing medical records confirming your diagnosis or signing a release form to request your medical records will be required. If you think you may qualify but have not been diagnosed then we may be able to refer you for diagnostic evaluation (please call or email to discuss). Once you've been evaluated and deemed an appropriate medical marijuana patient, you will immediately be covered by "affirmative defense". The next step will be to register you with the state pharmacy patient registration portal. This is due to open online once the state pharmacy has been notified by dispensaries that they will be opening within 60 days. The state pharmacy will not issue medical cards until they know patients can access a dispensary within 60 days of receiving their card. Patient's registered by our office will receive an email to complete their registration online. You will pay $50 for the state annual fee ($25 for disabled and veterans) and you can print off your medical card in real time. When the dispensaries open, you can gain access with the medical card and a state ID/driver's license.
